Pre-Admission Testing

Three Gables Surgery Center requires that all patients complete pre-admission testing prior to their procedure. Call 740.886.9911, ext. 4 to schedule.

 What to bring with you day of surgery:

• Driver’s license
• Insurance card
• Living will and/or medical power of attorney
• Any legal documents, such as custody or legal guardianship
• All surgical documents given to you by your surgeon

Request for Medical Records

Copies of medical records may be obtained Monday-Friday from 9 a.m.-4 p.m. (Closed legal holidays).

Please remember that only the patient and his/her legal representative are authorized to pick up records. Please call with any questions.

Smoking/Vaping Policy

Three Gables Surgery Center is a tobacco-free campus. Smoking and/or vaping are not permitted on the grounds.
